Information requirements

  • Your AICIS Business ID 
  • AICIS assessment certificate number
  • Consent by all joint certificate holders
  • Chemical name or AICIS approved chemical name (AACN)

After you submit your application 

We can add you as a certificate holder provided you have given us the required information and uploaded the relevant consent forms.

If your application meets requirements, you will be added as a certificate holder. We will advise you and all certificate holders by email.

Anyone added as a certificate holder must understand their compliance obligations.

How to submit your application

You must submit signed consent forms from certificate holders with your application.

Upload application and consent form(s) in AICIS Business Services

Step 1

Download the application form and fill out the required information including the declaration.

Step 2

Make sure you save it to your hard drive as a PDF. Click File > Save and select a location for the file. 

Step 3

Login to AICIS Business Services with your user name and password.

Step 4

Click on the 'Other applications' tab, then click on 'create new application'. From the drop down list of applications click 'Add a certificate holder'.

Step 5

If there are joint certificate holders, all of these must sign a consent form each and you need to upload each consent form with the application.

Step 6

Submit.

Note, we cannot process your application if we don't receive consent forms.

 

 

Note if you want to be removed as a holder of an assessment certificate and you are the only certificate holder – you need to apply to cancel a certificate on this page.

Once we process your application, we will advise you and all certificate holders, and you will not be able to introduce the chemical without further authorisation by AICIS.

Step 1

Step 6

Upload and submit your form.
